Medicare Part D Services

MediCall has a long and successful history in providing Medicare Part D clinical services. These services include:

• Medication Therapy Management (MTM) support for high quality, cost effective communications directly with beneficiaries as required by the new Medicare Part D Criteria.

• Inbound Information Line to support formulary, billing and coverage issues from beneficiaries
• Outbound Patient Discharge calls to members who have been recently discharged from the hospital to impact readmission rate
• Outbound Health Risk Assessment calls to members who may be "at risk" for deteriorating health issues and who could benefit from more focused case management services from a U.S. based clinical team
