Post Discharge Programs

MediCall has designed and implemented a variety of Post Discharge programs for many Healthcare companies. These programs have proven to be successful and have resulted in a reduction of hospital readmissions for our clients who have implemented them. Post Discharge programs can be for members who were hospitalized for any condition. We have developed scripts for general admissions as well as specific procedures/conditions.
